Improvement in health-related quality of life after therapy with omeprazole in patients with coronary artery disease and recurrent angina-like chest pain. A double-blind, placebo-controlled trial of the SF-36 survey
<p>Abstract</p> <p>Background</p> <p>Many patients with coronary artery disease (CAD) have overlapping gastroenterological causes of recurrent chest pain, mainly due to gastroesophageal reflux (GER) and aspirin-induced gastrointestinal tract damage. These symptoms can be alleviated by proton pump inhibitors (PPIs). The study addressed whether omeprazole treatment also affects general health-related quality of life (HRQL) in patients with CAD.</p> <p>Study</p> <p>48 patients with more than 50% narrowing of the coronary arteries on angiography without clinically overt gastrointestinal symptoms were studied. In a double-blind, placebo-controlled, cross-over study design, patients were randomized to take omeprazole 20 mg bid or a placebo for two weeks, and then crossed over to the other study arm. The SF-36 questionnaire was completed before treatment and again after two weeks of therapy.</p> <p>Results</p> <p>Patients treated with omeprazole in comparison to the subjects taking the placebo had significantly greater values for the SF-36 survey (which relates to both physical and mental health), as well as for bodily pain, general health perception, and physical health. In comparison to the baseline values, therapy with omeprazole led to a significant increase in the three summarized health components: total SF-36; physical and mental health; and in the following detailed health concept scores: physical functioning, limitations due to physical health problems, bodily pain and emotional well-being.</p> <p>Conclusions</p> <p>A double dose of omeprazole improved the general HRQL in patients with CAD without severe gastrointestinal symptoms more effectively than the placebo.</p
Choroba niedokrwienna jelit w praktyce lekarza rodzinnego
Choroba niedokrwienna jelit występuje w trzech postaciach: ostrego niedokrwienia
krezki, przewlekłego niedokrwienia krezki (CMI) oraz niedokrwienia okrężnicy. Ostre
niedokrwienie krezki jest trudnym do rozpoznania stanem klinicznym, o śmiertelności
sięgającej 59–93%. O jego rozpoznaniu często decyduje powiązanie silnego bólu brzucha
ze skąpymi objawami przedmiotowymi oraz współwystępowaniem czynników ryzyka
niedokrwienia okluzyjnego (np. migotania przedsionków) lub nieokluzyjnego
(np. wstrząsu). Wdrożenie leczenia chirurgicznego (resekcja jelita, endarterektomia,
by-pass) lub wewnątrznaczyniowego (selektywna fibrynoliza, trombektomia, angioplastyka,
stentowanie) przed pływem 24 godzin od początku objawów daje pacjentowi znacznie
większe szanse na przeżycie i uratowanie jelita. Przewlekłe niedokrwienie krezki jest
najrzadszą postacią choroby niedokrwiennej jelit, która klinicznie przejawia się poposiłkowymi
lub wysiłkowymi bólami brzucha, lękiem przed spożywaniem posiłku i znaczną
utratą masy ciała. O rozpoznaniu decydują wyniki badań obrazowych: USG doplerowskie,
angio-CT, angio-MR. Istotne zwężenia naczyń krezkowych w powiązaniu z objawami
klinicznymi jest podstawą do zaproponowania leczenia rewaskularyzacyjnego, głównie
metodą chirurgiczną. Obecnie do leczenia wewnątrznaczyniowego CMI, ze względu
na gorsze wyniki odległe, powinni być kwalifikowani jedynie pacjenci znacznie obciążeni,
o przewidywanym krótkim czasie przeżycia. Niedokrwienie okrężnicy to najczęstsza
i bardzo heterogenna postać choroby niedokrwiennej jelit, objawiająca się przeważnie
ostrym bólem brzucha i krwawieniem z okrężnicy. Postawą rozpoznania jest kolonoskopia,
a leczenie jest przeważnie zachowawcze.

Is rosuvastatin better than atorvastatin and simvastatin in the prevention of in-stent restenosis and atherosclerosis progression in patients after superficial femoral artery stenting due to chronic lower limb ischaemia? The preliminary case-control study
Introduction. Statins effect on the outcome of endovascular intervention due to chronic lower limb ischaemia (CLLI) is still uncertain. The aim of this study was to determine the effect of statin type on the late outcome of superficial femoral artery (SFA) stenting in patients with CLLI.
Material and methods. Retrospective analysis of the medical documentation of 275 consecutive patients treated with SFA stenting due to CLLI, including 125 (45%) patients with critical limb ischaemia (CLI). Measured outcomes were: target lesion revascularization (TLR), target extremity revascularization (TER), and target limb amputation (TLA).
Results. Statins were used by 267 (97%) of the patients, respectively: atorvastatin (n = 191, 70%), simvastatin (n = 31, 11%) and rosuvastatin (n = 45, 16%). During the 675.0 ± 569.7 days of follow-up, TLR was required by 79 (29%) patients, TER by 109 (39%), and TLA by 27 (10%). Patients treated with rosuvastatin in comparison with those treated with atorvastatin, in spite of greater initial LDL and triglyceride levels, required TER (p = 0.01) and TLR (p = 0.03) less frequently. The risk of TER in patients treated with rosuvastatin was significantly (p = 0.016) lower than in individuals treated with atorvastatin and simvastatin, as shown in the Kaplan-Meier analysis. Cox’s proportional hazards regression showed that therapy with rosuvastatin was the strongest factor (HR 0.40 ± 95% CI; 0.2-0.81) decreasing the likelihood of TER.
Conclusions. Rosuvastatin after SFA stenting seems to have the strongest effect on reduction in reintervention risk but without influence on limb salvage.
Zaburzenia żylne krążenia trzewnego w praktyce lekarza rodzinnego
Patologie układu żył trzewnych, podobnie jak żył kończyn dolnych, mogą polegać na ichzwężeniu, niedrożności lub refluksie. Najczęstszymi spośród nich są zakrzepica żyły wrotneji zakrzepica żył wątrobowych, czyli zespół Budda-Chiariego. Pierwsza może rozprzestrzeniaćsię na żyły krezkowe. Zakrzepica żyły wrotnej może przebiegać w postaci ostrej, którejtowarzyszą dość szybko narastające bóle brzucha, okolicy wątroby, czasem krwawieniez żylaków przełyku, niewielkie wodobrzusze lub objawy zawału jelita z typowym zespołemotrzewnowym. Podane objawy należy wiązać z zakrzepicą żyły wrotnej u pacjentówz tzw. czynnikami ryzyka zakrzepicy żylnej, takimi jak: marskość wątroby, zaburzenia systemowe(zespół mieloproliferacyjny, trombofilie, ciąża, połóg, antykoncepcja doustna) lubzaburzenia lokalne (zabieg operacyjny, nowotwór, stan zapalny w okolicy wnęki wątroby).Chorobę rozpoznaje się przeważnie w badaniu USG z Dopplerem, rzadziej potrzebnejest angio-TK lub angio-MR. Leczenie polega głównie na stosowaniu antykoagulacji doustnejwedług ogólnych zasad, rzadziej konieczne są metody wewnątrznaczyniowe.W postaci przewlekłej zakrzepicy wrotnej, gdy dochodzi do transformacji jamistej żyły wrotneji rozwija się nadciśnienie wrotne, terapia polega na pierwotnej i wtórnej profilaktycekrwawienia z żylaków przełyku i dna żołądka. Preferowane są metody endoskopowe, alemożliwe także wewnątrznaczyniowe i chirurgiczne, łącznie z przeszczepieniem wątroby.Zespół Budda-Chiariego ma podobne przyczyny jak zakrzepica żyły wrotnej, objawia siępodobnie, choć przebieg ma cięższy. Terapię dobiera się w zależności od stanu chorego,od antykoagulacji, poprzez selektywną trombolizę, stentowanie po przeszczepienie wątroby.Spośród żylnych zespołów uciskowych najbardziej znany jest zespół ucisku na żyłęnerkową lewą przebiegający pod maską zespołów gastroenterologicznych, nefrologicznych,ginekologicznych i urologicznych
Acute mesenteric ischaemia — is this the time for 24-h angiographic service? The role for endovascular therapy
Ostre niedokrwienie jelit jest heterogennym schorzeniem, względnie rzadkim, ale o bardzo poważnym rokowaniu.
Może być ono spowodowane niedrożnością (zator, zakrzep) lub skurczem tętnicy (niedokrwienie
nieokluzyjne), albo zakrzepicą żył trzewnych. Istnieje obecnie wiele przesłanek prognozujących wzrost epidemiologicznego
znaczenia tego zaburzenia, wynikający głównie ze starzenia się społeczeństwa i postępu
w leczeniu ciężkich stanów kardiologicznych. Krytyczne znaczenie rokownicze ma rozpoznanie i wdrożenie
leczenia ostrego niedokrwienia jelit przed upływem 24 godzin od początku objawów. Jest to możliwe jedynie
w przypadku uwzględnienia ostrych zaburzeń krążenia trzewnego w diagnostyce różnicowej ostrego bólu
brzucha, szczególnie jeżeli wystąpi on w związku z wieloma stanami klinicznymi sprzyjającymi upośledzeniu
perfuzji krezkowej (np. migotanie przedsionków, odwodnienie, przyjmowanie pewnych leków i substancji).
Kliniczne podejrzenie powinno być potwierdzone w angiografii metodą tomografii komputerowej lub rezonansu
magnetycznego. Na podstawie ich wyniku pacjenta kwalifikuje się do wykonania angiografii inwazyjnej i odpowiedniego
leczenia przezskórnego. W okluzyjnej postaci ostrego niedokrwienia jelit, oprócz nawodnienia,
proponuje się odessanie skrzepliny lub zatoru, selektywną trombolizę przez cewnik wprowadzony do tętnicy
krezkowej górnej, plastykę i/lub stentowanie oraz leczenie hybrydowe lub chirurgiczne. Leczenie przezskórne
uzupełnia się dotętniczym wlewem leku rozszerzającego naczynia, najczęściej papaweryny, oraz heparyny
dożylnie. Natomiast w postaci nieokluzyjnej ostrego niedokrwienia jelit, będącej skutkiem skurczu naczyń
trzewnych (np. w przebiegu wstrząsu), stosuje się tylko dotętniczą infuzję papaweryny. Leczenie to zmniejsza
śmiertelność pacjentów z ostrym niedokrwieniem o 18–53%. U pacjentów, u których dochodzi do martwicy
jelita lub występują objawy otrzewnowe, zaleca się wykonanie oszczędzającej resekcji jelita. U każdego
pacjenta, który przeżył ostry epizod ostrego niedokrwienia jelit, konieczna jest diagnostyka jego przyczyn
(np. zator sercopochodny, trombofilia) w celu wdrożenia profilaktyki wtórnej.
Acta Angiol 2011; 17, 4: 237–250Acute mesenteric ischaemia (AMI) is a clinically heterogenous, relatively seldom occurring disease, which is,
however, characterised by poor prognosis. It may be caused by mesenteric artery occlusion (by embolus or
thrombus) or spasm (non-occlusive form), or visceral vein thrombosis. Current observations point to an increase
in AMI incidence, mainly due to prolonged lifespan in the general population and improved treatment
outcomes in severe cardiovascular diseases. Making AMI diagnosis and starting treatment within 24 hours
from onset of symptoms is crucial for patient prognosis. This is feasible if disturbances of mesenteric circulation
are considered in differential diagnostics of acute abdominal pain, especially in cases with possible impairment
of mesenteric perfusion (e.g. atrial fibrillation, dehydration, use of certain substances or drugs). Clinical
suspicion should be verified by computer tomography or magnetic resonance angiography. Results of these
examinations lead to patient qualification for invasive angiography and an appropriate form of percutaneous
intervention. In occlusive AMI, clot or embolus aspiration (percutaneous thrombo-/embolectomy), selective
thrombolysis via catheter introduced into superior mesenteric artery, percutaneous angioplasty and/or stenting
as well as surgery or hybrid procedures are considered. Percutaneous treatment should be complemented
with intraarterial vasodilator, most often papaverine, and intravenous heparin infusions. In non-occlusive AMI,
caused by spasm of visceral arteries, e.g. in the course of shock, infusion of papaverine is the only treatment
applied. This approach decreases mortality due to AMI by 18–53%. In patients with bowel necrosis
and/or signs of peritoneal irritation, bowel sparing surgical resection is the procedure of choice. Each patient
surviving an AMI episode requires a detailed diagnostic work-up in order to find out the underlying cause
(e.g. cardiogenic embolus, thrombophilia) and set up an appropriate secondary prophylaxis strategy.

Fully conjugated [4] chrysaorene. Redox-coupled anion binding in a tetraradicaloid macrocycle
[4]Chrysaorene, a fully conjugated carbocyclic coronoid, is shown to be a low-bandgap π-conjugated system with a distinct open-shell character. The system shows good chemical stability and can be oxidized to well-defined radical cation and dication states. The cavity of [4]chrysaorene acts as an anion receptor toward halide ions with a particular selectivity toward iodides (Ka = 207 ± 6 M–1). The interplay between anion binding and redox chemistry is demonstrated using a 1H NMR analysis in solution. In particular, a well-resolved, paramagnetically shifted spectrum of the [4]chrysaorene radical cation is observed, providing evidence for the inner binding of the iodide. The radical cation–iodide adduct can be generated in thin solid films of [4] chrysaorene by simple exposure to diiodine vapor
Soluble vascular cell adhesion molecule-1 in patients with non-valvular atrial fibrillation
Background: Disturbances in atrial microcirculation is recognized as a risk factor for atrial fibrillation (AF).Aim: The aim of this study was to determine the associations between circulating soluble vascular cell adhesion molecule-1 (sVCAM-1) and the risk of AF and a one-year prognosis among consecutive inpatients.Methods: Eighty consecutive inpatients hospitalized due to non-valvular AF and 80 consecutive inpatients admitted for exacerbation of chronic coronary syndrome (control group) were enrolled in the study. A cardiologic workup was performed and blood sVCAM-1 concentration was determined using the ELISA method.Results: Patients with AF had similar blood sVCAM-1 concentration compared to the control group. AF patients treated with new oral anticoagulants (NOACs) were significantly less likely to have a sVCAM-1 concentration elevated above the median value than patients treated with warfarin (34.2% vs 65.8%; p = 0.01). Patients with an increased percentage of fat mass (FM) had lower sVCAM-1 concentration. The risk of all-cause mortality and MACE during follow-up rose in individuals with elevated sVCAM-1 (≥ 1242 and ≥ 587 ng/ml, respectively) with (OR; 95%CI): 5.39; 1.57-18.45; p = 0.007, and 6.00; 1.18-30.37; p = 0.03, respectively. Risk of death rose with increase in the ratio of sVCAM-1 and FM (1.02; 1.00-1.04; p = 0.019).Conclusions: Elevated sVCAM-1 was associated with all-cause mortality and MACE during one-year follow- up, but do not links the risk of AF. Use of NOACs may favorable affect endothelial function, A lower level of sVCAM-1 in obese patients may mediate the phenomenon of the “obesity paradox” in patients with AF

Mutational analysis in podocin-associated hereditary nephrotic syndrome in Polish patients: founder effect in the Kashubian population
Hereditary nephrotic syndrome is caused by mutations in a number of different genes, the most common being NPHS2. The aim of the study was to identify the spectrum of NPHS2 mutations in Polish patients with the disease. A total of 141 children with steroid-resistant nephrotic syndrome (SRNS) were enrolled in the study. Mutational analysis included the entire coding sequence and intron boundaries of the NPHS2 gene. Restriction fragment length polymorphism (RFLP) and TaqMan genotyping assay were applied to detect selected NPHS2 sequence variants in 575 population-matched controls. Twenty patients (14 %) had homozygous or compound heterozygous NPHS2 mutations, the most frequent being c.1032delT found in 11 children and p.R138Q found in four patients. Carriers of the c.1032delT allele were exclusively found in the Pomeranian (Kashubian) region, suggesting a founder effect origin. The 14 % NPHS2 gene mutation detection rate is similar to that observed in other populations. The heterogeneity of mutations detected in the studied group confirms the requirement of genetic testing the entire NPHS2 coding sequence in Polish patients, with the exception of Kashubs, who should be initially screened for the c.1032delT deletion
